This form is where most of the errors occur. It really is a simple form and the doctor fills out a lot of it so it should not be that much of an issue. Nevertheless, this is the form that gets rejected time and time again. First, you must make sure that the form is the most recent form. In order to find that, you look in the upper right hand corner and find the date. As of this writing, the required date is 7/25, but that date changes usually at least once a year, so always make sure to check the Hillsborough County Athletics page to know the most accurate date .
Next, the top of the form must be completed fully. It is not enough to simply put in the student's name and date of birth. The entire section must be completed.
In the middle section, there is a stamp that must be present and the little box must be checked in the left hand corner of that section (assuming there is no relevant medical history). Then, the bottom of the middle section must be signed and dated by both the student and parent/guardian. If there are any medical issues, the physician will leave the box unchecked and will write in the issues below. The physician may also choose to attach a supplemental page. He or she should know how to do that.
At the bottom of the page, any restrictions on participation must be noted by the physician or nurse. Then, the part that gets missed many times is that the physician must sign the section, put his or her credentials in and put in his or her license number. This is in addition to the stamp that goes into rectangular box in the bottom section. Many physicians do one or the other. There must be a stamp AND there must be a written signature, credentials and license number in order to have a valid form.
Again, Page 4 (and any medical issues written on the supplemental page) is the only page that should be uploaded into the EL2 section. Do not upload all four pages and do not upload the pages into the supplemental section of the Clearance paperwork site. That just makes it harder for us to find the necessary information.
