Page 4 of the EL2 form is the only page from your physical that needs to be uploaded.  Because this one causes so many problems, we have a section devoted solely to the EL2 form.

You need to upload an electrical bill, water bill or mortgage statement that is within the last 30 days.  That proof of residency is known as "living proof."  The living proof must match the address that you have on record with the school.  If you are renting, you must upload the entire lease agreement and the player's name must be on that agreement. 

Do not upload a driver license of the student.  The parent/guardian's government ID must be uploaded.

You cannot just upload your own private insurance card.  You must purchase insurance from Hillsborough County Public School Athletic Protection.  If it is your first time getting school insurance for athletics, you will have to create an account (click here to go straight to that page).  It costs $40 if you are only getting insurance for soccer (it is more if you are getting insurance for more than one sport).

If your birth certificate is not in English, you will not be cleared.  If you need to have your birth certificate translated, you can go to sites such as www.floridatranslate.com and pay a small fee to get a quick translation.  It must be a certified translation, not just a translation written out by someone in your family that knows English. There are many other websites out there that perform this service.  This particular one is just one that we know works.

You must rewatch videos every year that you tryout and must get updated certificates.  For this year, if your certificates are not dated May 15 or later, they are not valid.
